Important COVID-19 information-
• If you have been recently diagnosed with COVID-19 or are experiencing any COVID-19 symptoms– fever, cough, sore throat, shortness of breath, body aches, new onset of vomiting or diarrhea, or loss of taste/smell, contact your physician. DO NOT come to the facility.
• Masking will be determined by community COVID levels and may be required upon entering the facility.
• To prevent overcrowding, visitors/drivers will be asked to wait in the car until patient is ready for pickup.
• Patients will be screened prior to their visit and while at the facility.
